Productive Pathways Transitional Services, Inc. (PPTS) is a 501c3 nonprofit organization that provides case management and transitional services to at-risk youth who are entering or exiting out of a juvenile commitment facility/juvenile system and in need of guidance with transitioning back into the community. PPTS provides each youth with the transitional skill needed to become self- sufficient. Ensuring that every youth throughout the community will have the social, coping and educational skills they need to become productive members of today's society. 

Each youth is given the opportunity to connect with a transitional mentor to build a long-term relationship where the focus is on supporting the growth and development of the youth. PPTS goal is to not allow a youth to return back into the community and not been given the opportunity to succeed. PPTS is focused on preventing risky behavior and help at risk youth achieve their full potential along with positive youth development, youth development strategies, such as leadership training, social skills, coping skills, job training/job attainment skills, leadership skills, and educational skills. Any youth who are exiting out of a juvenile commitment facility must have maintained a positive reporting along with a successful completion.

  

In addition to our youth services, we also provide transitional services and life coaching to families and individuals with uncertainties about their dreams and goals, not knowing how to create a plan for their everyday life, feeling overwhelmed with uncertainties about life in general or how to become self-sufficient. Our transitional strategist provides strategy/ life coaching to families and individuals who are ready to put THEMSELVES first and get a plan in action.

The transitional process

Providing each at-risk youth with the transitional skills needed to become self-sufficent.

 Pre-Transitional Services - Is the transitioning stage when a youth is entering a juvenile commitment facility or the juvenile system. Pre-transition is preparing the youth to come back into the community. We will assist the youth to maintain the skills he or she has acquired while in a commitment facility. We will ensure that while the youth is in pre-transition, he or she is connected with a transitional mentor. 


The transitional mentor will guide he or she in their development stage in which it is allowing the youth to work towards gaining social skills, coping skills, job training/job attainment skills, leadership skills, and educational skills needed to become a productive members of today's society. Also, this allows the transitional mentor and youth to build a long-term relationship where the focus is on supporting the growth and development of the youth.

Such as:

  • Helping the youth see the positive outcomes ahead while creating a detailed map about how to get there
  • Sharing of the youth goals and fears openly
  • Not expecting the transitional mentor to solve the youth short-term or long-term problems or do the work for the youth
  • Discussing the areas where the youth is struggling or failing
  • Listening, researching and applying the transitional mentor's guidance, etc.
  • Directing the youth on how to maintain independent living and training skills.



  




We encourage every youth to maintain an open mind on their role and what is their end goal. PPTS focuses on the results which is for the youth to become a positive, productive, self-sufficient member of society.

Also, we will ensure that the youth is connected with a transitional mentor. Their transitional mentor will guide them in their development stage in which the youth will work towards gaining social skills, coping skills, job training/job attainment skills, leadership skills, and educational skills needed to become productive members of today's society. Also, this allows the youth to recognize how to build positive long-term relationships.  





Transition starts when an individual is ready. What if you had the opportunity to expose your gift or what you're good at? In order to be clear on your gift and what you're good at, you must be clear with YOU. If whatever you want to do scares you, it might be a good thing to try. Transition is never easy but the good thing is that we can start right where we're at and DO IT!
